Dana Reisch, LMSW, CMC

Dana has worked in the field of geriatric care management for 12 years with specializations in dementia, behavioral healthcare, and stroke care.

Dana has had a psychotherapy private practice specializing in geriatric patients, and she has served as an Adjunct Professor at Hunter College's Brookdale Center for Healthy Aging. Additionally, she ran the geriatric care management program for Caring People Inc., worked for Village Care of New York's nursing and rehabilitation center, and worked for New York Presbyterian's inpatient psychiatric program.

Dana received her Masters of Social Work from The State University of New York at Stony Brook.

Sheila Kolt, LCSW, CMC

Sheila has worked in the field of geriatric care management for almost 15 years with specializations in dementia, behavioral health, stroke, spinal cord injuries, and multiple sclerosis.

Sheila worked as Director of Care and Relationship Management at Alliance Homecare and was responsible for managing the clinicians and bridging care managers to a large network of industry professionals. Sheila oversaw and ensured excellence in delivery of quality care for the company. Prior to Alliance, she worked as Director of Resident Services at Brookdale Senior Living, in which she managed care for almost 200 residents. For 6 years, Sheila was Senior Geriatric Care Manager at SeniorBridge, Humana and that is where her spark commenced in the care management field. She worked as a social worker in a private psychiatric hospital, Holliswood Hospital. Sheila built and managed all aspects of her part time private practice in psychotherapy in conjunction with a psychiatrist.

Sheila is a member of the Aging Life Care Professional Association since 2006 and serves as Treasurer on the Board (NY Chapter). She is a member of the Case Management Society of America and ORION Resource Group, an organization dedicated to ensure that older adults thrive in the community.

Sheila graduated from the University of Pennsylvania with a Bachelor of Arts in Psychology and Columbia University with a Masters in Social Work.
